Summary

Adds branch diff mode syntax (-b <branch> [--base <base>]) to the README Usage section
Adds brief explanation of how branch diff mode works (full file scan, default base branch)

Context

PR #1 added branch diff mode and updated SKILL.md and CHANGELOG.md, but the README.md usage section was not updated. This PR fills that gap so users discover the feature from the README.

Test plan

Verified the new usage examples match the syntax in SKILL.md
Confirmed formatting renders correctly in GitHub markdown preview
