Track Numbers not being applied properly to file names or metadata (numbers exist but are incorrect *may be true to original source but not the downloading album/playlist)
this is particularly bad when downloading playlists. but also happens with albums

can the track number on filename or tag (or preferably both, but either will do) follow playlist order/position?
