Sync ProxyPilot with upstream CLIProxyAPI releases v6.8.40 through v6.8.46.

watcher debounce and immediate-update fixes

ProxyPilot tracking updates

update UPSTREAM_VERSION to v6.8.46
update features.json upstream marker to CLIProxyAPI v6.8.46
add a session note for the sync and release tag
tag the release as v0.3.0-dev-0.40

Verification

go build ./cmd/server ./cmd/proxypilot-tray ./cmd/proxypilotpack
go test ./...

one pre-existing failure remains on clean origin/main and on this sync branch
