Refactored #596

Changes:

Changed URLs both in czechia and Slovakia.md to sktv.plainrock127.xyz which is my self-hosted SKTV Forwarders instance
Fixed urls from #596, where the requester (don't know how to name it) included links to main page instead of stream url.
Added Východočeská TV (EPG: V1.cz), TV Nova (EPG: TVNOVA.cz) and ÚTV / ÚSTECKÁ TV (EPG: UTV.cz)
Removed Minimax CEE since it had URL of Retro Music Station
